Failing to identify previously unknown risks and mitigate the risks known to the operation increases the likelihood of an accident. Member Graham will present a National Transportation Safety Board (NTSB) case study demonstrating the importance of operators identifying, managing, and mitigating the risks of their operation. This case study will also highlight the need for operations of all sizes to implement an effective safety management system.
